The Alliance for Community Health was founded in 2012 by a group of physicians committed to enabling all children and families to receive the care they need. For years, our physicians have known that their patients need connectivity to services in the community. We are here to help.

Our goal is to never duplicate services in the community. Rather, we assemble partners and work together to connect or expand services.

Community Health Workers
Community Health Workers are often on the front lines for our fight for health equity. They can be the right person at the right time that connects individuals to the care or the community resources they need.

Our programs require funding to make a difference. Every dollar helps us go the extra mile for people we serve. The Alliance for Community Health is a non-profit organization according to the laws of the Internal Revenue Service. All donations to the organization are tax deductible.
